Family Navigator

I am a psychology student at the University of Western Australia, with a strong interest in understanding people, emotions, and how support systems can make a meaningful difference in times of stress or transition.

I have worked across a range of customer service and hospitality roles, supporting people from diverse backgrounds. These experiences have helped me develop strong communication skills, patience, and an ability to listen carefully and respond with respect and clarity.

Outside of work and study, I value health and wellbeing, enjoy travelling, and spend as much time as possible by the beach.

How a Family Navigator can help:

Navigating mental health services can feel confusing or overwhelming, especially when you are already carrying a lot. The Family Navigator is here to help you make sense of the available support and guide you through your options in a calm, respectful, and non-pressured manner.

The Family Navigator supports you to understand the services offered at Discovery Family Therapy and to explore which options may best suit your needs. This includes explaining the different types of support available, what each service involves, and how they may align with what you are looking for.

When helpful, the Family Navigator may join an initial session to gain a better understanding of your situation and what matters most to you. From there, they help identify services (both within Discovery Family Therapy and externally) that align with the clinic’s values of care, safety, and respect.

The Family Navigator does not make decisions on your behalf. Instead, they provide clear information, thoughtful guidance, and trusted options, allowing you to make contact and take next steps when you feel ready.

How I can help

  • Acting as a Family Navigator, helping families understand their options, coordinate care between counsellors, and feel supported throughout their time with Discovery Family Therapy.

  • Helping you access external supports and services, including community and allied health resources.
     

  • Assisting you to navigate the NDIS, including helping gather and organise documentation for submissions.

Specialising In

  • Helping clients access external supports and services outside of therapy, such as employment, housing, or community resources (where appropriate).
     

  • Acting as a personal service finder, helping you understand what is available and how to access it.

Working with a Family Navigator means they don’t have to hold everything on their own.

What is unique about Yasmin as a Family Navigator?

I am naturally empathetic and a great listener, which helps people feel understood and supported. 

I also have a logical and practical way of thinking, which enables me to help others make sense of challenges and move forward in realistic ways.

Most impressive achievements

At the age of 15, my family relocated to Queensland for my dad’s work. During that year, I changed schools, built new friendships, moved house several times, and found my first job. Navigating so much change at a young age helped me grow significantly as a person and develop a strong sense of independence, adaptability, and resilience. I then moved back to Perth and rebuilt my teenage life here. 

Throughout my schooling and university studies, I am proud to have consistently achieved strong academic results.

One quirky fact

I can’t cook, but I can make perfect poached eggs!
